Solar elastosis is prevalent in skin biopsies from FAPD as the diffuse thinning predisposes to actinic damage (Figure 15.5).

PEARLS

In a patient with FFA, always look on the parietal scalp to exclude FAPD; in case of uncertainty – take a trichoscopy-guided biopsy.

Distinguishing FAPD from AGA with perifollicular fibroplasia and inflammation (“complicated AGA”) sometimes is not feasible and this should be noted in the report. As a rule, in FAPD there is an onion-like zone of pale/mucinous fibrosis and thinned follicular epithelium, lichenoid inflammation (vs. spongiosis with lymphocytic infiltrate in AGA (see Chapter 7).
